Applying to be re-credentialed

Your Mental Health Nurse Credential is valid for three years after which time you must apply to be re-credentialed. To maintain the continuity of your credentialed status, you should submit your re-credentialing application 3 months in advance of the expiry date of your current credential. The ACMHN will send you a notice reminding you of the expiry date of your credential in advance.

 

Step 1: Read the Application Guidelines

You should read the Application Guidelines for re-credentialing before you start completing the Application Form:

Step 2: Complete your Application

 We recommend you use the Word version of the Application Form which has been designed for you to save to your computer and complete electronically. When you have completed the Application, please print and sign it. Please do not use a handwritten format for this form.

Step 3: Complete your Evidence Based Record

As part of your application, you will also need complete an Evidence Based Record (EBR) for the past three years. An EBR contains details of professional activities you undertake as part of your continuing professional education and continuing practice development as a mental health nurse.

Step 4: Obtain two References

You application needs to be supported by professional referees.
