CREDENTIAL FOR PRACTICE PROGRAM
APPLYING FOR CREDENTIALING
To commence your application, download the Application Form, the Evidence Based Record Guidelines and Guidelines for Referees.† Read these Guidelines and the Application Form carefully. The Guidelines provide explanatory notes for specific questions. Please complete all questions as directed. Applications that do not follow the Guidelines may not be accepted or will be returned for further information and this will delay timely processing.
Please ensure you are using the correct form when you complete your application. Only the form currently on the webpage will be accepted. This can be identified by the version number at the bottom of the first page. For an initial application the current version is V12 8/04/2008.
Where a copy of a document is required to be certified, it must be certified by one of the following people only:: Legal Practitioner (e.g. lawyer), Justice of the Peace, Commissioner of Declarations, Commissioner of Oaths, Judge, Magistrate.
If you are applying under the provision for equivalence, you will also need to ensure your application is submitted as per the current guidelines on the website.

You should read the entire Application and Guidelines before you start. If you have require help after you have read the all the information on credentialing, please contact the ACMHN on 1300 667 079.
If you are involved in the Mental Health Nurse Incentive Program, you may submit your application even if you have not completed all the criteria. Just ensure you provide a cover letter to explain this. You will then be provided with advice on what you need to do to meet the criteria.
When you submit your application, please make sure all required documents are attached as requested. When your application is received at ACMHN, you will receive notification. Your application will be processed within 6-8 weeks and this will depend on the completeness of your application.
